A geriatric care manager is a health care professional such as a nurse, gerontologist, social worker or counselor who specializes in assisting older people and their families to attain the highest quality of life and cope with the challenges of aging.  The professional geriatric care manager has extensive knowledge about costs, quality, and availability of community resources.  In addition to promoting the highest standards of practice, the geriatric care manager is committed to maximizing the independence and autonomy of elders, and strives to ensure the highest quality and most efficient health and human services. 

Focus On Senior Care, LLC., offers a wide variety of compassionate, personalized services focusing on short term or ongoing assistance based on the individual's wants and needs.  We customize all services to meet the needs of the client, family and support team.  These include, but are not limited to:

  • Conducting mini as well as comprehensive care planning assessments to identify problems and provide solutions. Meeting with the client/s and family members together or separately (as desired) to determine clients level of functioning and review all health care issues. 
  • Providing liaison services to families at a distance
  • Providing coordination of care for client/s: manage medications, arrange and monitor in-home assistance, arrange and attend physician visits with client/s, provide communication to physicians, family members and support persons, provide in-home safety check and recommend necessary safety changes to keep client/s safe, and identify any and all problems on a continual basis. 
  • Interpreting, translating and advocating for client
  • Providing consumer education
  • Providing assistance with relocation and placement services
  • Providing assistance with a move, setup, and adapting to new environment

    • Insurance agents
    • Discharge planners
    • Mental health professionals
    • Physicians
    • Nursing facilities
    • Law enforcement
    • Retirement residences
    • Trust officers and guardians
